Community Alternatives Program for Disabled Adults (CAP/DA)

Clay County Health Department Community Alternatives Program for Disabled Adults (CAP/DA) is a special community care program. CAP/DA makes care at home a real possibility for many elderly or disabled adults who face nursing home placement or wish to return home from a long-term care facility.

With CAP services, along with support from family and friends, many individuals can remain or return safely home at a lower cost than nursing facility care. CAP exists to supplement rather than replace the formal and informal services and support already available to individuals. Our main objective is the allow eligible clients to remain in or return to the community and live as independently as possible. Eligibility requirements apply.
